Senior Clinical Pharmacist (Maternity Cover)

Senior GP Practice Clinical Pharmacist - (Maternity Cover)
TheHurley Group is looking to welcome an experienced Pharmacist who will workwithin the senior pharmacy team on a maternity cover basis to support and develop the optimisation ofmedicines management at our Staunton Group Practice in London; by workingwithin the general practice setting and remote online setting: eConsult (anonline remote consultation toolhttps://econsult.net).
Thisrole will be part of the general practice team to provide specialist advice topatients, and undertake full patient centered medication reviews to proactivelymanage people with complex polypharmacy, particularly the elderly and thosewith multiple conditions.
Main duties of the job

Work in general practice will include service design, medicines reconciliation, management of prescribing queries and acute medication issues following hospital discharges, and the development and implementation of safe systems for medication ordering and prescribing in the context of improving safety and reducing waste.

Job responsibilities

POLICY/SERVICE DEVELOPMENT
·Develop virtual and remote models of consultation andsupport including e-consultations, remote medication review and telehealth andtelemedicine
·To work in a virtual/remote setting to workwith general practice staff to promote and embed the role of a GP practicepharmacist, and raise awareness of services available to patients.
·To provide general practice staff acrossmultiple GP practice sites with recommendations and plan action to proactivelyimprove the quality of systems and processes toimprove medicines optimisation within the general practice setting.
·To provide trainingregarding medication related issues and policies/guidelines to general practicestaff in order to promote medicines optimisation, repeat dispensing and highquality cost-effective prescribing as appropriate.
·To provideoperational and clinical support to medicines optimisation developments andprescribing initiatives as dictated by GP practice, CCG, and hospital Trust andservice objectives.
·To collate and feedback on electronic dischargerelated information to ensure continuing improvement of the hospitalcomputerised discharge letter and process of communication.
·To assist with the implementationof policies and guidelines to improve medicines optimisation across primary/secondary care interface and into the general practice setting.
·To provide trainingto general practice staff to promote medicines optimisation in practice.
·Develop and deliver patient facing services andclinics which improve patient outcomes, reflect the needs of the practicepopulation and shift workload from other disciplines
·Deliver high quality medication reviews anddevelop bespoke medicines treatment plans for patients (particularly theelderly, complex patients and those at high risk of admission) to reduce riskand unplanned hospital admissions
·Contribute to the achievement of KPIs andobjectives of the programme
·Work with the general practice team to ensurethe practice is compliant with CQC standards, leading improvement initiativeswhere medicines are involved
·Work within the eHub to process medication eConsults,reconcile medications and process medication workflow across the Hurley Groupprimary care sites
PATIENT/CLIENT CARE
·To act as a specialist practitioner in GeneralPractice
·To promote and deliver pharmaceutical care ingeneral practice and promote safe pharmaceutical management across theinterface
·Providepro-active leadership on medicines and prescribing systems to the practicemultidisciplinary team, patients and their carers
·Toprovide highly specialised evidence-based clinical advice and information tonursing and medical staff on the care and treatment of patients
·To liaise with other pharmacists to ensureefficient transfer of clinical and medicines management issues
·Contributeto patient safety culture through Significant Event Reporting and investigationof medicines related incidents and through pro-active measures
·Ensurethat patients are referred to the most appropriate healthcare professional forthe appropriate level of care within in an appropriate period of time
FINANCIAL/RESOURCE MANAGEMENT
·To identify areas of potential cost-savings andto implement systems to realise savings and contain prescribing cost
·To develop and implement policies andguidelines to control drug expenditure and ensure cost-effective prescribing.
·To educate other healthcare professionals onthe costs associated with drug therapy and promote the use of evidence-based,cost-effective prescribing within the division
·To attend financial meetings
STAFF MANAGEMENT & TRAINING
·Superviseand direct the Band 7 Clinical Pharmacists in their rotational roles within theHurley group and Kings Health Partners
·Activelyparticipate in national training scheme for clinical pharmacists
To participate in the recruitmentand selection of pharmacy students, pre-registration pharmacists, andjunior pharmacists as needed.
·Toassist in the training, supervision and assessment of junior pharmacy staffproviding a clinical pharmacy service
·Toplan activities for, supervise and train pre-registration pharmacists andpharmacy students where appropriate
·Tosupervise and support the day to day activities and progress through nationaltraining scheme of senior GP practice pharmacists
To participate in the in-serviceclinical pharmacy training of all pharmacy staff.
To undertake CPD to ensure thatknowledge is continually updated
To participate in the training anddevelopment of medical, nursing and other healthcarestaff, as appropriate
To actively develop an AdvancedPractice Portfolio mapped to the RPS Advanced Practice Framework with aview to RPS Faculty recognition
To act as a role model forpharmacists developing advanced practice skills
RESEARCH & DEVELOPMENT
To regularly initiate or contributeto relevant practice research or audit projects and to publish the resultsof such work, as appropriate.
To participate in other researchand audit programmes in the departmentas required
To maintain and further developspecialist information on the use of drugs in clinical conditions relevantto the specialty.
To publish and present the resultsand outcomes of own research and audits locally, nationally orinternationally as appropriate
Person Specification

Experience

5 years qualified with appropriate post registration experience in hospital or GP practice pharmacy or equivalent area of practice with assessed competencies
Previous clinical pharmacy experience in relevant area
Previous clinical pharmacy experience in Acute and/or GP Practice Experience of supervising others
Experience of practice research or audit
Experience of protocol/guideline Experience of delivering training
Experience of directorate work
Qualifications

Registered Pharmacist MRPharmS
Mandatory CPD to maintain fitness to practice
Postgraduate Diploma in Clinical Pharmacy or Pharmacy Practice
Independent Prescribing Qualification in which accreditation will be used to support management and outcomes for patients
