Job Title: Senior Clinical Pharmacist Band 8a

Employing Organisation: Haringey GP Federation

Hours of work (WTE): Full time (37.5 hours per week)

Contract: Permanent

Remuneration: Dependent on experience

Accountable to: Band 8b Lead Clinical Pharmacist

Responsible to: Director of Operations, Haringey GP Federation

Main duties of the job
The post holder is an experienced prescribing pharmacist who acts within their professional boundaries. They will work as part of a multidisciplinary team in a patient-facing role and lead on various workstreams for their practice and PCN, including:

Chronic disease management

Structured medication reviews for target groups

Improving prescribing safety through a proactive approach on high-risk drugs, safety alerts, and CQC compliance

The post holder will provide 7 sessions of clinical pharmacy time to a local primary care network in Haringey and 3 management sessions at the Haringey GP Federation headquarters.

Job responsibilities
Long-term conditions
Review patients in multimorbidity clinics and in partnership with primary healthcare colleagues, implementing improvements to patients' medicines, including deprescribing.

Manage own caseload and run long-term condition clinics. Prescribe independently for conditions with a large medication component (e.g., respiratory, cardiometabolic, renal diseases).

Review ongoing medication needs, monitoring requirements, and support patients in medication management to optimize their use of medicines.

Structured Medication Reviews
Undertake SMRs for specific patient groups (e.g., multimorbidity, polypharmacy, frailty, care home residents), revisiting annually.

Resolve issues autonomously, including deprescribing and ordering relevant tests.

Care home/residential clinical medication reviews
Conduct medication reviews for care home residents, collaborating with local PCN care home leads, and improving MDT communication.

Medicines reconciliation and management
Reconcile medicines post-discharge from hospital, identify unexplained changes, and work with patients and pharmacists to ensure continuity of care.

Clinical advice and expertise
Respond to medicine-related inquiries, signpost to community pharmacy, and ensure adherence to SOPs and safety standards.

Antibiotic Stewardship
Promote evidence-based antimicrobial prescribing and work with leads to reduce inappropriate use.

Repeat Prescribing
Support management of repeat prescriptions, review requests, and monitor medicines as an independent prescriber.

Quality Improvement projects
Lead and participate in clinical audits and QI initiatives.

Training & supervision
Support development of pharmacy staff, including training and supervision aligned with local and national priorities.

Person Specification
Experience

Minimum of 3 years post-qualification experience, including general practice setting

Familiarity with GP systems

Accountability for professional expertise and service delivery

Experience in motivational interviewing and staff training

Qualifications

Masters in pharmacy (MPharm) or equivalent

Registered with the GPhC

Post-graduate clinical diploma or equivalent

Membership or progress towards membership of the Royal Pharmaceutical Society
