The post holder will work as part of the Ansdell Medical Centre Medicines Management team supporting the GPs, the Practice Senior Pharmacy Technician and the Medicines Management Assistants.
The post holder will provide specialist medicines advice and technical assistance to implement and maintain high quality, cost effective prescribing and reduce medicine related risk to patients.
The post holder will support the development of a safe, cost effective, efficient practice repeat prescribing system through reviewing, improving and implementing medicines management processes in liaison with the medicines management team.
The post holder will be responsible for helping the practice to deliver the agreed changes required to meet prescribing targets set for the practice.
The post holder will also be a contact point for medication related queries from pharmacies, nursing homes, and other health care professionals, to improve the accuracy of patient medication records and help deal with any patient-related medication queries as appropriate.
The post holder will be responsible for other allocated tasks such as clinical audits as required and will liaise closely with the GPs to co-ordinate Medicines Management work plans.

About us Ansdell Medical Centre is looking for a fun, friendly, and focused individual to join our Medicine Management team. The hours will be up to 30 per week.
Job responsibilities Carry out work to identify patients suitable for medication changes in line with the Medicines Management work plan to support QOF, prescribing targets, and ICB projects. Subsequently, make those changes with the GP's agreement, e.g., dose optimisation, and formulary switching.
Assess prescribing data using practice clinical computer systems and patient records, highlight any recommendations and implement changes if required, to ensure patients medication records are up to date.
Liaise with patients, practice staff, GPs, care home staff and community pharmacists in all aspects of medication reviews and prescribing queries to improve the pharmaceutical care of patients.
Support medication review and medicines reconciliation. Work to gather information to support the practice pharmacist/doctor/nurse carrying out medication reviews or clinical audits.
Implement initiatives to reduce waste and improve medicines compliance.
Implement service changes relating to prescribing to ensure best practice is adopted in accordance with local guidelines, legislation, safety updates e.g NICE, MHRA
Under the direction of the GPs, assist with medication changes from hospital letters and medicines reconciliation for new patients.
Highlight any medication errors/queries and report any concerns to the GPs to support local clinical governance structures eg datix reporting.
To assist the GPs with any searches, data collection and letter production required for prescribing changes or drug alerts.
To provide information and answer technical medicines management queries from healthcare professionals and members of the public.
Identify potential issues/areas, which require work to achieve maximum cost or clinical effectiveness in prescribing
In conjunction with the medicines management assistants, be responsible for ensuring recall systems are in place, and working, for the correct monitoring of high-risk medicines.
To assist the medicines management assistants to screen online prescription requests and queries to ensure they are dealt with according to practice policy.
To help develop and implement medicines management initiatives through the work of the practice e.g. repeat dispensing, pharmacy MURs.
To cover the general duties off the medicines management assistant when on holiday to generate repeat and acute prescription requests on the surgery computer system and process repeat prescriptions daily ensuring compliance with practice protocols.
Provide training for practice staff on repeat prescribing systems and help develop any required prescribing protocols.
In conjunction with the GPs deliver training and education on prescribing and therapeutics in general practice.
Work in a professional manner, complying with confidentiality and data protection policies.
Person Specification Qualifications BTEC in pharmaceutical sciences / NVQ level 3 or equivalent.
Registered with the General Pharmaceutical Council.
Evidence of continuing professional development.
Experience of working with GPs.
Enrolled on or qualified from Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH).
Post qualification experience in community pharmacy, secondary care or primary care.
