Ouraim is to provide high-quality, patient-centred care in the comfort of theirown homes, reducing hospital admissions and length of stay in hospital andenhancing overall well-being through enhanced care coordination.

Main duties of the job
As Dietician, you will play a critical role in our newmultidisciplinary team, the 6 newfrailty virtual wards that we have planned for our locality. The post holderwill work closely with consultants, pharmacists, ACPs and other healthcareprofessionals to ensure seamless, integrated care on the virtual ward, byproviding specialist ACP assessments and interventions for those admitted tothe Frailty Virtual Ward.
Key Responsibilities
To provide a specialist, high-quality nutritionand dietetic service within Frailty Virtual Wards.
To provide specialist dietetic assessment,diagnosis, treatment and education to individual patients with on the FrailtyVirtual Ward.
To provide advice, signposting, guidance andinformation to health and social care professionals, patients, carers,relatives or other non-professional contacts
To be an integral member of the MDT providingeducation, training and dissemination of information to all healthcareprofessionals.
To be an autonomous practitioner, holding aclinical caseload and working without direct supervision
To continue own professional development in linewith HCPC regulations
A secondment will only be considered if your current manager is supportive of you undertaking a secondment. A secondment may have the possibility of becoming permanent

To work as the lead / sole dietitian for Livewells Frailty Virtual Wards, with support from the wider MDT and clinical Leads from the Frailty Virtual Ward (line manager once in post). The post sits within the Adult Frailty, and Specialist Services Directorate.

Experience
Essential
Competence with patients with complex needs, communication issues and/or challenging behaviour. Varied and comprehensive post-graduate clinical experience across a range of clinical areas. Experience of working in a multi-disciplinary team. Experience of delivering in-service training. Experience of interpreting and acting on complex clinical information including anthropometry, biochemistry, pharmacology and medical history to make decisions about client care. Evidence of participation in audit activities in dietetic practice. Experience and competence in negotiating and recording aims of treatment and agreeing a plan of action with client/relatives/patients/medical staff.
Desirable
Competence and experience in advanced communication skills including negotiation, motivational interviewing and evoking behavioural change in complex cases. Knowledge and Experience of enteral feeding and re-feeding syndrome as well as alternative methods of nutritional support.
Knowledge
Essential
Demonstrate specialist knowledge of Best Dietetic Practice in the treatment or management of nutritional needs. Evidence of an accurate up to date portfolio in order to demonstrate Continued Professional Development (CPD) lifelong learning and skills of reflective practice. Demonstrate ability to deliver best practice for a range of more complex clinical conditions. Computer literacy skills, including quick and accurate use of keyboard, presentation of data, word processing, electronic delivery of presentations report writing and internet use to inform practice. Knowledge and understanding of supporting patients with rehabilitation of neurological impairments.
Desirable
Membership of Specialist Interest Group. Member of British Dietetic Association. Previous experience in working within neuro rehabilitation.

Qualifications
Essential
BSc in Human Nutrition and Dietetics or related qualification and 3 or more years demonstrable relevant experience. HCPC Registration. Evidence of on-going professional development.
Desirable
Evidence of further formal study leading to postgraduate qualifications Diploma in Advanced Dietetic Practice, Masters degree. Working towards or hold a certificate in counselling skills or have extensive counselling experience. PENG clinical update. A teaching qualification or experience of teaching groups. Clinical Supervisory Skills Course.
