Working Hours: Monday to Friday, 9:00 AM to 5:00 PM, with a 30-minute break

Job Description:

We are currently seeking a skilled and dedicated Clinical Pharmacist with experience in Renal Wards to join a well-established healthcare team in East London/Essex. This is a fantastic opportunity for an experienced professional to provide high-quality pharmaceutical care to patients on the renal wards within a busy hospital setting.

Key Responsibilities:

Provide clinical pharmacy services to patients on renal wards
Work closely with multidisciplinary teams to optimize patient care
Review and dispense prescriptions, ensuring compliance with all clinical guidelines
Advise on the safe and effective use of medications
Support renal team in managing patient drug regimens
Perform clinical checks, counselling, and education for patients
Maintain accurate patient records and ensure proper documentation of medication-related activities

Requirements:

Hospital Experience is essential
Recent experience working in renal care or a similar specialty is highly desirable
Must be registered with the General Pharmaceutical Council (GPhC)
Strong communication and interpersonal skills
Ability to work effectively within a team and independently
Experience working Monday to Friday, 9am-5pm shifts
