• Hours: Full-time, 37.5 hours per week (Monday to Friday)
  
  About the Role:
  Trident Healthcare is seeking a highly skilled and motivated Clinical Pharmacist to join our dynamic team. This is a fantastic opportunity to contribute to patient care in a hospital ward-based setting, working closely with multidisciplinary teams to ensure safe and effective use of medicines.
  
  Key Responsibilities:
  
  
• Conduct comprehensive drug histories and medicines reconciliation for newly admitted patients
  
  
• Perform clinical screening and TTA (To Take Away) screening
  
  
• Provide expert pharmaceutical advice and support to ward staff
  
  
• Ensure accurate and timely documentation using Electronic Prescribing and Medicines Administration (EPMA) systems
  
  
• Collaborate with healthcare professionals to optimise patient outcomes
  
  Essential Requirements:
  
  
• NHS experience in a hospital setting
  
  
• Proven hospital ward-based experience including drug history, medicines reconciliation, clinical screening, and TTA screening
  
  
• EPMA experience is essential – ideally with Cerner, the system used at CHS
  
  
• Excellent communication and organisational skills
  
  
• Ability to work independently and as part of a team
