Job overview

This is an opportunity for an experienced clinical pharmacist to be involved in a charitable funds approved post-discharge counselling project.

The project involves identifying suitable patients during their inpatient stay and providing a follow up counselling appointment post-discharge.

The project aims to identify patients on high risk medication and ensure they can make the best use of their medications once they have left the hospital.

Main duties of the job

We are looking for someone with vast ward based experience and a passion in this area of work within hospital pharmacy. You must be an enthusiastic, motivated and a patient/staff orientated pharmacist dedicated to patient safety.
You will need to have excellent communication skills and enjoy working as part of a large multi-disciplinary team. You will also need to be friendly, approachable, adaptable, flexible, willing to learn new skills and be committed to continuous professional development.

You will be a pharmacist registered with the General Pharmaceutical Council and have significant clinical experience.

This is a fixed term post for one year to support the charitable funds project.

The post involves:

• Contacting patients who have been discharged from our Trust
• Providing counselling on a range of medication
• Signposting to primary care or other appropriate services
• Supporting data collection for project evaluation
• Presenting findings within the pharmacy and wider as required
• Working closely with the existing project team to ensure the project is delivering on all KPIs
• Communicating with ward teams on potential inpatient interventions where identified

Person specification

• Member of the General Pharmaceutical Council
• Diploma in Pharmacy Practice or equivalent
• MSC
• Previous publications in national journals
• Previous experience as a tutor or mentor
• Wide range of ward pharmacy experience
• Training, mentorship, supervision of staff
• Excellent communication, presentation, interpersonal skills
• Ability to manage workload
• Evidence of time management skills
• Management of people, systems and change
