The practice pharmacist offers expertise on medicines and health to help support patients, as well as the rest of the clinical team. Pharmacists monitor the use of medications as well as their efficacy and safety. The role of an independent prescriber adds a range of benefits including being able to prescribe any medication within their competence.

The prescribing pharmacist plays an integral role in the management of long term conditions such as diabetes. They will be supported at all points by a GP.

Islington Central Medical Centre is a large teaching andtraining practice with almost 25,000 patients and a large team of clinical andadministrative staff. ICMC is a purpose-built surgery, located just a 5-minutewalk from Highbury and Islington Station, which has excellent underground,overground and national rail transportation links.

Main duties of the job

  • Ensuring that the repeated scripts are signed and sent in a timely manner
  • Conducting medication reviews
  • Managing chronic diseases
  • Dispensing prescriptions
  • Medication queries, education of patients and medical counselling
  • Monitor patients medical therapies
  • Provide exemplary care in accordance with current legislations and requirements
  • Working closely with the clinical team
  • Providing quality and personalised care to patients

You will be working solely at ICMC but employed by N2 Islington PCN Site ICMC

The practice pharmacist must:

  • provide exemplary care
  • provide support and advice to the other members of the MDT
  • advise the practice of the safe management of medicines
  • assess the wellbeing of patients in regard to their medication
  • have competency across a range of pharmaceutical subjects supported by professional and clinical knowledge
  • contribute to patient safety culture
  • help lead person centred medicines optimisation
  • promote patient involvement in their care
  • participate in regular appraisals and attend training and development activities
  • maintain a non-judgemental attitude
  • uphold patient confidentiality
  • plan and manage and monitor and review general care plans for patients with chronic conditions

Accountable to: GP Partners and Practice Manager

Person SpecificationQualifications

  • Masters in Pharmacy or equivalent
  • Member of General Pharmaceutical Council
  • Independent Prescribing Qualifications
  • Member of Primary Care Pharmacy Association
  • Member of Royal Pharmaceutical Society

Skills

  • Works well in teams/MDTs
  • Strong clinical skills and knowledge
  • Communicates well
  • Passion and dedication to improving patient care

Experience

  • - 2 years minimum experience as a pharmacist
  • - Worked in a pharmacy previously
  • - Experience in triaging patients
  • - Experience in handling blood results
  • - Experience in handling e-consultations
