As a leader of theinpatient multi-disciplinary team at Dunes ward, Hartley Hospital,you will work in a state-of-the-art mental health facility,providing clinical care to older adults who require specialistinpatient care. You will work closely with a wide range of clinicaland professional disciplines to deliver holistic care by completingassessments of new admissions to Dunes ward and managing theirtreatment, ensuring safe discharges back to the community. Mainduties of the job The post holder will be responsible forcompleting assessments and formulating treatment plans for serviceusers admitted to Dunes ward from the community. Dunes ward is a20-bedded primarily functional older adult complex care ward atHartley Hospital. As a substantive Old Age Psychiatry Consultant onDunes ward, you will work closely with other members of themultidisciplinary team. You will be the Responsible Clinician forpatients admitted to Dunes ward, managing patients who have beendetained under the Mental Health Act, requiring a sound knowledgeof Mental Health Act law. This includes completing relevant mentalhealth act documentation and capacity assessments as required. Thepost holder will be expected to lead the multidisciplinary team atDunes ward in an enthusiastic, supportive, and approachable mannerto foster positive working relationships and morale among wardstaff. The post holder will complete ward rounds throughout theweek and provide clear communication and documentation regardingassessment outcomes and management plans. They will also provideleadership and training to trainee doctors and medical studentsplaced on Dunes ward, taking an active part in the clinical work ofthe team and a lead role in the development of the service. Detailed job description andmain responsibilities Regular reviews of inpatients to monitor theresponse to treatments will be required, and the post holder willoperate within multi-disciplinary integrated and co-located teams,including Health staff and Social services staff. They will beexpected to liaise closely with Social Services to ensure safe andtimely discharge planning of inpatients and communicate promptly toprimary care services by providing discharge summaries. At times,patients under General Adult teams in the community may be admittedto Dunes ward if this is deemed the most appropriate place for themat the time due to bed availability in the Trust. The post holderwill be expected to provide inpatient care through assessment andtreatment as required while such patients await transfer to moresuitable inpatient units, if necessary.
