Tollgate Medical Centre is one of three practices that formDocklands PCN. Individually, we providePrimary Care Services to around 18,500 patients, and our PCN provides care andsupport to over 40,000.

We are a strong practice, and are proud of the work we dowithin our PCN. We have an expandingmultidisciplinary team including GPs, Nurses, HCA, Pharmacists, SocialPrescribers, Care Coordinators, Paramedic, First contact Physiotherapists, andothers. We are a training practice andunderstand well the need to support and nurture staff, as well as encouragingand providing the necessary training to help you achieve your goals.

Job responsibilities

The post holder is an experienced general practicepharmacist, who acts within their professional boundaries, supported andworking alongside the team at Tollgate Medical Centre.

Working as part of a multi-disciplinary team, the postholder will provide clinical pharmacy leadership for the practice, takingresponsibility for, and not limited to, Structured Medication Reviews, areas ofchronic disease management, medicines optimisation, quality improvement, thequality and outcomes framework and other enhanced services.

The post holder will be responsiblefor leading, training and coordinating the work of professionals within theorganisation, to proactively and competently. The post holder willcompetently manage repeat prescribing systems, proactively dealing with acute andrepeat prescription requests and medicines reconciliation on transfer ofcare. Where necessary the role wouldinvolve putting in place systems for safer prescribing, providing expertise inclinical medicines advice while addressing both public and social care needs ofpatients in the GP practice.

The post holder will provide support to the practice staffwith regards to prescription and medication queries, ensuring integration withcommunity and hospital pharmacy to help utilise skill mix, improve patientoutcomes, ensure better access to healthcare and help manage GP workload.

You will work closely with colleagues within the Practiceand the PCN to ensure adherence to prescribing formularies, and medicinesoptimisation programmes, whilst maintaining patient safety. Regular meetings within the PCN, communitypharmacists, and the ICB Pharmacy team are essential to share learning and tobe up-to-date with new guidelines and targets.

The role is pivotal to improving quality of care andoperational efficiencies and so requires motivation and passion to deliverexcellent service within general practice and the wider PCN.

Experience

  • Working as a pharmacist in General Practice full time (or equivalent) for more than 2 years as a Practice Based Pharmacist
  • Experience of: teaching & training, clinical supervision and mentoring and coaching for change
  • Management of self and others to achieve goals and targets
  • Leadership of other healthcare professionals, particularly pharmacists
  • Experience of working across a range of pharmacy settings
  • Experience of leading and developing multidisciplinary clinical teams
