Blackstone Recruitment is working with a Medical Practice in Torquay that are looking for a Pharmacist to permanently join the practice. This is an exciting opportunity for a Clinical Pharmacist to work with 2 forward thinking, innovative practices across Torquay as they continue the exciting journey developing a PCN.

This is a permanent position.

Job Summary

  • Band 7 to 8 equivalent salary for the right candidate, subject to experience and qualification

  • Hours: 37.5 hours per week, Monday-Friday (no weekend working)

  • Contract: Full-time, Permanent

  • You will join a team that is looking to positively impact the health and well-being of a 26,000 plus patient population.

  • We welcome applications from pharmacists working in all sectors of pharmacy.

  • You will work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.

  • You will take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities (in particular frailty).

  • You will provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at the PCNs practice(s) and to help in tackling inequalities.

  • Provide leadership on person centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services.

  • Through structured medication reviews, clinical pharmacists will support patients to take their medications to get the best from them, reduce waste and promote self-care.

  • Developing relationships and working closely with other pharmacy professionals across PCNs and the wider health and social care system, you will take a central role in the clinical aspects of shared care protocols, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties), liaison with community pharmacists, secondary care and care homes.

    Job Specifics

    Rate: £42,000 to £50,000 per annum

    Working Hours: Monday to Friday (no weekend working)

    Contract Type: Permanent

    Essential Recruitments

  • Masters Degree in Pharmacy (MPharm) or equivalent.

  • Registration with General Pharmaceutical Council.

  • A minimum of 2 years post‐qualification experience

  • Whilst not essential, knowledge of EMIS, Word and Excel would be an advantage as would experience of working within Primary Care.

  • This role involves an element of travel between PCN sites as and when required.
