Job overview You will need to demonstrate a person-centred approach and have effective interpersonal and organisational skills. You must be a pharmacist registered with the General Pharmaceutical Council (GPhC) and have the ability to work independently and as part of a multidisciplinary team. The successful candidate will be working with the wider multidisciplinary team of nurses, doctors and therapists.
The successful applicant may have contact with patients or service users. As an NHS Trust we strongly encourage and support vaccination as this remains the best way to protect yourself, your family, your colleagues and of course patients and service users when working on our healthcare settings.
Main duties of the job The post is offered as an excellent opportunity for band 6 to 7 progression. Progression is subject to completion of necessary in-house criteria and a successful portfolio review. It also offers a great opportunity to develop your clinical and professional knowledge and skills. Salary will be based on previous experience, applicants who are currently suitable to practice at a band 7 level, will be considered for a band 7 position.
Working for our organisation We have an exciting opportunity for a motivated and enthusiastic pharmacist to work in our community intermediate care unit, in Hillingdon, which provides rehabilitation care for patients requiring short-term inpatient care.
Detailed job description and main responsibilities Applicants who are registered pharmacists, with experience in hospital pharmacy, and a strong interest in care of the elderly/intermediate care are encouraged to apply for this post.
Person specification EDUCATION AND QUALIFICATIONS Vocational Master’s Degree in Pharmacy (4 years or equivalent)
Registered with the General Pharmaceutical Council (GPhC)
Working towards post graduate certificate/diploma in Clinical Pharmacy Practice
Working towards membership of the faculty of the RPS
PREVIOUS EXPERIENCE Demonstrate the ability to appropriately recommend, substantiate and communicate medicine related information to patients, carers and clinical staff
Previous evaluated experience of mentorship and training skills
Recent significant and relevant experience in a Hospital Pharmacy
Experience in clinical pharmacy for community health services/ elderly people/frailty/rehab/
Experience of Quality Improvement
SKILLS, KNOWLEDGE, ABILITIES The ability to identify and prioritise clinical work load
Communicates with medical, nursing, and other MDT & pharmacy staff in clear precise and appropriate manner.
Clinical and critical appraisal skills
Evidence of writing procedures, guidelines or protocols.
PERSONAL CHARACTERISTICS Demonstrates ability to meet set targets
Demonstrate ability to organise self in order to ensure efficient use of time.
Meets expected levels of practice as defined by others
Advanced clinical knowledge in healthcare for older people
Has an awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice.
